FORMS & POLICIES

Back to School

School forms are available below and/or will be distributed digitally to parents on an as-needed basis. However, there are a few paper-based exceptions: forms that require physician/dentist signatures, including health, medication, and dental; these must be downloaded, completed, signed, and returned to our administrative office via mail.

  • RETURNING STUDENTS

    You will receive an email with Summit’s Parent Contact Information Form attached for your review and reply with approval and/or changes. We respectfully request that school forms be received at Summit prior to August 31. One exception (for children receiving prescribed medication): our online Medication Information Form is requested before August 17. An email-based reference copy of any Summit online form will be sent to the person completing the form once the form is submitted.

    Step 1: If your child receives any prescribed medication, whether at home or in school, or might need prescribed emergency medication (EpiPen, inhaler, etc.), please complete online Summit’s Medication Information Form before August 17. Having this information early in our planning is especially important.
    For children who must receive medication in school administered by our school nurse, you must also download and print our Medication Form [PDF] for your physician to sign and return to the school nurse (contact info below) before August 31. Medication can only be administered at school with signed instructions from a physician.
    Step 2:
    Review and submit your reply with approval and/or changes for Summit’s Parent Contact Information Form which will be emailed to all returning Summit families (for all students). Questions: Contact Colleen Hopkins at chopkins@summitqueens.com.
    Step 3: Join/Rejoin Summit’s Parents’ Association online and volunteer at school events (for all parents).
    Step 4: Download and print at home Health & Dental forms in the Medical Forms tab (for all students per detailed instructions below).
    Step 5: Download and print Summit’s 2023/2024 School Calendar [PDF] for your reference (for all parents). All school events will be announced in Summit’s eBlast, our email-based newsletter, as information becomes available.

  • NEW STUDENTS

    Please complete online school forms and requested medical forms. We respectfully request that school forms be received at Summit prior to August 31. We suggest that 7-10 minutes be allowed to complete our online Parent Contact Information Form, which can be completed for up to two students. This form must be completed and submitted during one session; work cannot be saved and continued in multiple sessions.
    Step 3: Complete and submit the online Release Form.
    Step 4: Download and print NYCDOE Medicaid Reimbursement Form [PDF] (for new students who reside in NYC only). Complete in writing, sign, and return to Summit’s administrative office.
    Step 5: Join Summit’s Parents’ Association online and volunteer at school events (for all parents).
    Step 6: Download and print at home Health & Dental Forms in the Medical Forms tab (for all students per detailed instructions below).

  • MEDICAL FORMS

    Medical/Health, Medication, OT
    Prescription & Dental Form [FOR ALL STUDENTS]

    Prior to the beginning of each school year, parents must submit the required medical forms for their children: Health Examination Form, Medication Form (if administered by the school nurse), and Dental Health Form. It should be noted that OT prescriptions (if mandated on NYC IEPs) will be coordinated in September. All of these forms are paper-based as physician/dentist signatures are required, and links are provided for parents to download and print at home.

    All medical forms are due before August 31.

    Health Examination Form
    •  Required for new admissions and before entering grades 3, 5, 7, 9, and 11
    •  Required annually for students who participate in team sports
    •  Must be signed by a physician

    Occupational Therapy Prescription Form (For students with OT mandates who reside in NYC only)
    •  Must be completed each school year and signed by a physician; personalized student forms will be emailed to parents in September once available from the NYCDOE
    • This form is required to be on file by NYS Medicaid for OT services to be provided
